Lost Policy Receipt
A form that a Life insurance policyholder will obtain from the insurer, which he or she will sign in order to surrender a lost policy. The signed form becomes a receipt that indicates the policy is no longer in effect, which protects the insurer if a policyholder later says that the policy was never surrendered.
